RISE Community Wellness Program Participant Requirements
Home Residence
Live in the Northend, Pleasant City, or Coleman Park Communities
Age Requirement
18 years of age or older
Family members/caregivers will be encouraged to participate in program activities (but if under 18, they will not be included in the program nor will they be eligible for gift cards and other items for participation.)
Chronic Condition
All participants must have been diagnosed by a physician or other health care provider with one or more of the following conditions or through our Pre-Project Health Biomarker Screening Assessment have one of the following score findings:
Diabetes, Prediabetes, or Gestational Diabetes
An A1c Score of

5.7% to 6.4% – Prediabetes Range
-
6.5 % or higher – Diabetes Range
Heart Disease, including:
Coronary Artery Disease (CAD)
High Blood Pressure
A Blood Pressure Score of:
-
130 or higher– Systolic mmHg (upper number)
-
80 or higher – Diastolic mmHg (lower number)
High Cholesterol and/or Triglycerides
Cholesterol Score of:
-
> (greater than) 200
Triglycerides Score of:
-
> (greater than) 149
High Body Mass Index (BMI) Score
-
25 to 29.9 – Overweight
-
30 or greater – Obese
Mental Health – Emotional, Psychological, and Social Well-being
Not every resident will be impacted by these diseases/conditions, however, many may also be experiencing individual and/or family mental/emotional health issues.
